You may wish to have a nominated person to have access to your medication records. This can be complete access of access to certain parts. People do this for a variety of reasons including language barriers, working schedules or if they are your primary carer for example. If you wish to nominate someone to access your results or book appointments for you, please download the form and return it to reception once completed.
